Forms for Housing Providers

Use the Request for Tenancy Approval when you decide that you wish to rent a particular apartment to a Section 8 voucher holder.

HUD requires a standard Housing Assistance Payment (HAP) Contract for each Section 8 apartment under the program.

While you may use a lease of your choice, we offer a Model Lease for rental property owners who do not have their own.

If you decide you want to have your monthly Housing Assistance Payment directly deposited in your bank account, please complete and return the ACH form, with a voided check, to us.
